Droneographer release form?

There are LOTS of photography release forms online. Some of them have valuable information which carries over into droneography. But none have any reference to being filmed via drone.

Does anyone have a droneographer release form? One that preferably was drawn up by a team of nasty shark lawyers.

There's an app for iOS called Legal Flyer. It's not the product of lawyers, but does have some conveniences for what you're looking to do (I believe).

Thank you very much for sharing. I googled it and it looks awesome! Only I'm screwed because I am an Android guy. It looks like it's PC friendly though.

All of these various features/benefits from SDKs prompted me to add a Shield tablet to my iPad & iPhone arsenal. Now I'm an iOS AND Android guy!

On a side note @Ascender does an android/iphone guy record in .mov or .mp4? My androids don't like .mov's
